· (For student who is not well) When did you start feeling unwell? How do you feel? Do you have fever, sore throat, cough, runny nose, headache, etc?
· Forgot password to IVLE or faces technical issues, email to rvhs@moe.edu.sg, stating their name, class and NRIC number as well as phone number. School will respond to them ASAP.
· If the student has travelled to an H1N1 affected country/region and is not feeling well, staff will convey the following information to the students: “Please call 993 for medical assistance. You should also stay at home until you are fully recovered. Please do not go out e.g. shopping, for meals, take the MRT or socialise with others until the doctor tells you that you are well.”
· If the student has travelled to an H1N1 affected country/region and arrived in Singapore on or after 22 Jun 09, staff will explain to him/her that he/she will be issued with an LOA (Travel to Affected Countries) for the next 7 days from the date of return to Singapore (the 7 days exclude the day of return). During this period, he/she will stay at home, and the school will make arrangement for him to catch up on his/her school work. He/She will monitor his/her health and update the school on a daily basis. School will proceed to mail the LOA (Travel to Affected Countries) letter to this student. The student will arrange for the Travel Declaration Form to be returned to the school, or bring it back after the LOA period.

· submit travel declaration form via email rvhs@moe.edu.sg if they have travelled to affected area in the past 7 days.
· submit travel declaration form on the first day of school to be handed to form teacher.
· bring ODT (thermometer) to school EVERYDAY.
